For many patients, hearing the words “root canal” can be scary. They may have heard stories about the pain and difficulties of the treatment. However, the truth is that a root canal should not be painful and is no more difficult than having a filling. In fact, a root canal can relieve painful symptoms caused by a serious infection and can save the tooth. Millions of teeth are saved each year through root canal treatment, which repairs and saves infected and/or decayed teeth.

A root canal procedure can offer several benefits to your oral health. It can restore the chewing and biting force of the tooth, provide restored sensation, and offer a more natural appearance while protecting the tooth from further damage or infection. We will keep a record of your treatment and decide on the type of restoration necessary for your tooth. Typically, a filling or a crown is used to seal off the inside of the tooth.
A retreatment is typically done if the original root canal therapy fails.
Some signs you might need a retreatment include:
  • New tooth decay
  • New crack in the treated tooth
  • Leaking filling material or cracked crown
  • Undetected complex canal structures
  • Saliva entering the restored tooth
  • Narrow or curved canals not treated during the original procedure
  • There was a delay when placing restorative devices after the procedure
It is normal to feel some tenderness in the area over the next few days as your body starts to heal itself. You may also feel some tenderness in your jaw from keeping it open for an extended period of time. These symptoms are temporary and usually respond very well to over-the-counter pain medications. It is also very important to follow the post-op instructions (LINK) provided to you by your doctor. Your tooth may continue to feel slightly different from your other teeth for some time after your root canal treatment has been completed. However, if you have severe pain, do not hesitate to contact us.
You will receive detailed post-operation instructions and home-care guidelines after treatments. It’s also important to carefully follow the instructions for medications if prescribed. Some tenderness and soreness in the area of treatment after procedures are normal. In situations where the crown of the tooth is heavily damaged or decayed, a root canal treatment may include rebuilding the tooth interior with composite and a crown to restore it to optimal appearance and function. After your procedure, you should wait until the numbness in your mouth wears off before eating. Don’t chew or bite down on the treated tooth until it is fully restored by your dentist. Take care of your teeth by brushing, flossing, regular checkups and cleanings to maintain your teeth for lifetime.
You may need root canal when the nerve or pulp of the tooth becomes infected and inflamed due to dental decay, a cracked or broken tooth, or an injury to the tooth. The most common symptoms are mild to severe toothache, with teeth sensitivity to pressure, hot or cold foods or drinks. The purpose of the treatment is to completely remove the infected nerve and pulp and, seal up the tooth, and restore it to natural function. Your dentist may do root canal treatment or an endodontist. An endodontist is a dentist who has completed a university post-graduate specialty program in endodontics. Endodontics is a specialty of dentistry concerned with the treatment of the dental pulp or nerve of the tooth. If your child’s primary (baby) tooth is damaged, your dentist may refer you to a pediatric dentist for this procedure. A pediatric dentist has at least 2 years of extra university training in treating children.
Root canal treatment may be done in 1 or 2 appointments. After root canal treatment, your tooth may be tender for the first week or two. Bad pain or swelling are NOT common. If this happens, call your dentist or endodontist. You can still get a cavity or gum disease after a root canal treatment. Root canal treatment does not protect your tooth from other types of damage. With proper care and regular dental visits, the tooth could last as long as your other teeth. Most of the time, a tooth that has had a root canal treatment can be saved. However, there are cases where everything possible has been done to save a tooth and still the tooth must be extracted (pulled).
Sometimes root canal surgery is needed when a regular root canal treatment cannot be done or when it has not worked.
Surgery is done to:
  • Check the end of the root for fractures (cracks)
  • Remove parts of the root that could not be cleaned during regular root canal treatment
  • Clear up an infection that did not heal after regular treatment
Though it depends on the severity of the tooth in need of treatment, a root canal can be completed in as little as only one appointment. If the tooth is severely infected, several visits may be required to complete the work of properly cleaning and preparing the tooth root. On average, a root canal treatment can be completed within one to three appointments. During your initial consult, we are able to help you get a timeframe for how long the process will take, and that is a great time to discuss any concerns or questions with us.
With proper care, even teeth that have had root canal treatment can last a lifetime. But sometimes, a tooth that has been treated doesn’t heal properly and can become painful or diseased months or even years after treatment. If your tooth failed to heal or develops new problems, you have a second chance.

Dr. Liang, Diplomate, American Board of Oral Implantology, attended the University of British Columbia and received his Bachelor's degree in Microbiology in 1981 and his Doctor of Dental Medicine degree in 1983. At graduation, he was awarded the Canadian Oral and Maxillo-Facial Prize for highest standing in the area of Oral Surgery.

Dr. Tsang attended UBC where he studied mathematics before heading to Philadelphia to study at Temple University School of Dentistry. Dr. Tsang currently practices general dentistry in Abbotsford. He joins Dr. Liang and his team as an associate and implant dentist. As a life-long learner, he participates in hundreds of hours of continuing education each year.
